Enrollment Center Closing Information

• The enrollment center will be closed from Dec 22 - Jan 4th

• To book an appointment parents must complete the online registration located here. Once completed, it will prompt you to schedule an appointment with the Enrollment Center after the break.

• To speed along the process please make sure you have the following documents for your scheduled appointment time.

(We strongly encourage you to upload documents that were prompted to help expedite the enrollment process.)

• Original Birth Certificate.

• Immunization Record.

• Proof of Residency.
  1. Lease or mortgage in parent/guardian's name.
  2. Two different utility bills (Gas/Electric/Water/Phone) dated within 30 days in the parent/guardian's name.

Once the appointment is scheduled, it will provide you with an email address to send the following documents prior to your appointment:

• Parent/Guardian Government-Issued ID

• Any documents that you were not able to upload to the application

• IEP (if applicable)

• Custody Papers (if applicable)
