Just a quick reminder...

 

All 12th graders are required by the Ohio Revised Code #3313.671 and our Board Policy to show proof of the Meningococcal vaccination prior to entry into 12th grade. If the first dose was administered prior to the 16th birthday, then a second dose is required. If the first dose was administered after the 16th birthday, then 1 dose is required. Please provide an updated immunization record to the school nurse. 

Heart of Ohio Family Health is available at Whitehall-Yearling High School, Monday-Friday from 8 a.m.-5 p.m. for these vaccines and any other healthcare needs. You can walk-in OR make an appointment by calling 614-701-0980. All insurances and no insurance accepted. We offer a sliding fee scale to those who qualify. The health center is open to all and provides comprehensive family healthcare including: vaccines, sports physicals, sick visits, diabetes management, women’s health services, and more.
